Disability.gov Guide to Transition Planning


The transition planning process should begin around middle school and continue throughout high school. The student, his or her parents or guardians, teachers and school counselors should work together to develop a plan for life after high school. This plan should take into account the student’s strengths, preferences and interests, as well as any accommodations needs and other key factors.
